Before you start classes, you may need to submit immunization records.

Massachusetts law requires certain students to provide proof of immunization. Don’t stress. We’ll break it down.

Do I Need to Submit Immunization Records?

You must submit immunization records if you are:

  • A full-time student (12 or more credits) under age 30
  • Enrolled in any Health Science program, full-time or part-time
  • Attending on a student visa or other visa, including exchange or visiting students

If none of these apply to you, you likely do not need to submit records.

Below are the important immunization forms you’ll need:

Return immunization records to the Student Immunization Records Office.

Health Science and Allied Health Studies

All students enrolled in Allied Health programs including Nursing, Dental, Diagnostic Tech, Medical Assistant, Certified Nurse Aide, Medical Interpreter, and Paramedic are required to submit a physician’s certification that the student has received the required immunizations:
